Bonnie Mohan

Bonnie Mohan is a top leader and expert on the intersection of New York’s healthcare and housing systems.
For nearly two decades, Bonnie has worked tirelessly to integrate these systems to better serve New Yorkers with complex health and housing needs. She began her career in affordable housing at the University Neighborhood Housing Program (UNHP) in the Northwest Bronx, moved on to homeless services at BronxWorks, and then made the leap to health care, serving as Assistant Director of the Bronx Lebanon Health Home during its implementation.
In 2011, Bonnie tapped her unique, cross-systems perspective to co-found the Bronx Health & Housing Consortium.
Under her leadership as Executive Director, the Consortium has expanded from a grassroots convening in the Bronx to a citywide organization that reaches 4,000 individuals from 300 organizations annually. Bonnie has shepherded the Consortium through this growth while cultivating innovative partnerships and projects, expanding training offerings in response to the needs of the sector, and emerging as a go-to thought leader on health and housing in New York City.
Bonnie serves on the CAPS Steering Committee, Anthos|Home Advisory Committee, and NYU Langone COVID-19 Hotel Intervention for People Experiencing Homelessness (CHIP) Study Advisory Board. She has expertise in medical respite, supportive housing, NYC homeless services, and health home programs. She holds a BA in Urban Studies and Political Science from Fordham University and served for two years in Ethiopia in the United States Peace Corps.
Tess Sommer

Tess Sommer joined the Consortium in 2020, initially managing the organization's trainings and events. In 2024, she assumed the role of Director of Strategy and Engagement, where she contributes to the strategic growth of the organization, manages special projects, and oversees the engagement and expansion of the Consortium’s membership and larger network.
During her time at the Consortium, Tess has driven the expansion of the training program, launched steering committees to elevate the perspectives of frontline workers and people with lived experience into the organization’s work, and partnered with a wide range organizations to develop programs and practices to help meet the needs of New Yorkers.
Tess holds a Master’s in Urban Policy and Leadership from Hunter College, bringing a background in health policy and domestic violence research. Her prior experience includes roles as a Research Consultant at Safe Horizon, Community Manager of Body Politic–a New York-based queer feminist wellness collective–and a Program Manager for a community center. She holds undergraduate degrees in Political Science and Environmental Science from San Diego State University.
Jonathan Jimenez

Jonathan Jimenez is a dedicated social work professional with a strong background in case management, workforce development, and adult education. He holds a Master of Social Work (MSW) and has worked extensively across nonprofit, academic, and justice system settings, supporting diverse and underserved populations.
Jonathan's career has included roles as a Clinical Case Coordinator at CASES, Program Coordinator at Catholic Charities, and Academic Advisor at New Jersey City University. He has led trainings for frontline staff, supervised adjunct faculty, and developed curriculum focused on immigration services, legal system navigation, and trauma-informed care. His work emphasizes equity, empowerment, and cross-sector collaboration.
Currently serving as Training & Events Manager at the Health and Housing Consortium, Jonathan is passionate about creating engaging learning environments that build community capacity and promote social justice. He is also a long-time mentor to first-generation graduate students and a firm believer in the power of education, empathy, and systems change.

Committees

The Program and Policy Committee informs and supports the programmatic arm of the Consortium, including trainings for frontline and direct service staff and cross-sector events to facilitate collaboration and communication across sectors. It also helps synthesize members’ experiences to inform Consortium public policy strategies and actions. This committee organizes and activates members to take public actions in alignment with Consortium priorities.

The Consumer Advisory Committee is composed of people with lived experience of homelessness, housing instability, substance use, and/or incarceration. The CAC supports the Consortium by assisting staff in identifying training needs, evaluating policy priorities and making recommendations, and participating in panel conversations and other speaking engagements.
