Social Care Services for Supportive Housing Residents: Information Session with PHS

Please join The Health & Housing Consortium and the Supportive Housing Network of New York for this virtual information session on March 30, 10–11am, to learn how to connect your supportive housing tenants to health-related social needs (HRSN) services. These include nutrition support, transportation, care management, and housing services such as temporary rental arrears assistance and tenancy sustaining services. 

Representatives from the WholeYouNYC Social Care Network—serving Brooklyn, Manhattan, and Queens—will join us to overview the available services, eligibility criteria, and walk through the 12-question self-screening tool. We will then discuss opportunities for WYNYC staff to come to your sites and provide screening and navigation to your tenants.

With only one year remaining on the current Medicaid 1115 waiver, we must take advantage of these services while they are available and demonstrate the impact of Medicaid investments in social care needs. 

Additionally, there is a short window of opportunity to ensure members are screened and connected to services, which directly informs the State’s risk adjustment methodology and future funding allocations. Increasing screening and service uptake now is critical to accurately reflect the level of need across communities and to help sustain and scale these services beyond the current waiver period.

We encourage all supportive housing providers with buildings in Manhattan, Queens, and Brooklyn to attend this session. Please share this with any staff, such as program or site directors, who could potentially support a screening event at their site.
