Training: Multi-Disciplinary Coordination for People with Complex Needs

People with complex needs often face overlapping medical, behavioral health, housing, and social challenges that no single provider or system can address alone.

This training explores how these needs, combined with barriers to accessing services, require a coordinated response. Attendees will learn strategies to strengthen collaboration across healthcare, housing, and homeless services, build multidisciplinary teams, and develop shared, person-centered plans that reduce gaps in care and support continuity of services.

By the end of this training, attendees will be able to:

  1. Identify complex needs and understand how overlapping challenges and system barriers affect access to care, services, and housing.

  2. Describe the roles of key system partners and strategies to improve communication, collaboration, and shared accountability with care teams.

  3. Apply person-centered coordination strategies to develop shared action plans, address service gaps, and support transitions across systems.
