Training: Writing S.M.A.R.T. Person-Centered Service Plans

This training provides frontline staff and supervisors with practical strategies for developing meaningful, person-centered service plans that reflect each participant’s strengths, preferences, needs, and goals. Participants will learn how to apply the S.M.A.R.T. framework (Specific, Measurable, Achievable, Relevant, and Time-Bound) to create clear, actionable goals that promote engagement, accountability, and positive outcomes. The training emphasizes collaborative goal setting, participant self-determination, effective documentation practices, and ongoing progress monitoring. Supervisors will also explore strategies for coaching staff and ensuring service plans remain person-centered and outcome-focused.
